Position: Trusted Assessor (Live-in Care Team)
Salary: £650 per week plus travel expenses
Do you want to be part of an exciting new team of assessors working with our Live-in Service?
Are you well organised with excellent IT skills and more importantly enjoy caring for people?
Abicare needs a team of trusted assessors to be available to work throughout the country.
You will be the first person to meet the live-in client and introduce the Abicare live-in service to the client and their family. Reassuring them how the service works, answering any of their questions and concerns.
You will be required to cover the remote care package, which involves living with the client for 1-2 weeks until the new carer comes into post.
You will ensure that each client has a Service Delivery Plan which is person-centred and outcome-based and a full risk assessment.
Set up the Abicare tablet in order that all client notes, care plans, risk assessments and communication notes are digitally recorded. Set up the client hub and explain how this works and the benefits with the client and family.
Liaise with relevant caring professionals such as GPs, District Nurses as necessary and ensure all healthcare professional visits are recorded on the client’s digital care plan.
You will work closely with the client to match a suitable ongoing carer and complete a full detailed handover with the permanent live-in carer (covering all P&P’s with new carer).
You will have:
Previous experience working within the social care sector and qualified to Diploma Level 3
Excellent administrative, organisational and IT skills
The ability to work alone and be part of a team
Excellent communication and interpersonal skills
Problem-solving and the ability to stay calm under pressure
